To my esteemed patients:
Due to my relocation to open a practice in North Carolina, I must inform you of my intent to close my orthopedic practice, at Clark Sports Medicine in Henderson, on February 26, 2017. I will not be able to provide you with medical care after that date. Dr. Dan Fabito, a pain management specialist, will be occupying the office space treating painful spine conditions once I have departed.
I recommend you find another physician to take care of your orthopedic issues. Dr. Roger Fontes (Desert Orthopedic Center) has demonstrated willingness to see my patients for continuity of care and to store their medical records. Dr. Fontes is a fellowship-trained shoulder surgeon but has a broad orthopedic practice including hip and knee arthroscopy and replacement. I’ve referred him several patients over the years and he gets excellent reviews from my patients and great results. He may be contacted at:

As you are free to see other physicians, you may contact the Clark County Medical Society, Dignity Hospital System, the Yellow Pages or other resources for physicians to provide orthopedic care.
You may wish to obtain copies of your medical records while we are still open. You may come to my office to pick them up between now and February 23, 2017. Please give an advance call to my staff to allow them time to print your records prior to your arrival. Delivering your medical records to anyone other than yourself will require a signed written authorization from you.
As my practice outside of Wilmington, NC has not yet been set up, I am directing any specific correspondence for me to the address below after March 10, 2017. Given that this is a local office address yet to open, I can make no guarantee as to the promptness that I will receive correspondence.
